Records are stored in the website’s hosted database. A hash of your network address is used to limit repeated submissions. Hosting services process requests to operate the site. The forms do not expose submitted records to other visitors.

Use the contact form to ask about information you submitted or to request correction or deletion. Do not include sensitive information when making that request.
